Diabetic Specialist Nurses (DSNs) – Provide holistic and individual patient assessment, education to empower self-management skills, medication optimisation, initiation of insulin and other injectable treatments. Support diabetes technology and insulin pump therapy and along with DSDs provide a pre-pregnancy service and deliver structured patient education (DESMOND and DAFNE).
The team provide support and education to community nurses, hospice and care home staff and to other HCPs seeking advice.
Diabetes Specialist Dietitians (DSD)- Provide individual, specifically tailored, practical evidence based dietary advice. Patients are seen in a variety of different settings including outpatient clinics, community clinics, care homes and patients own homes. They see patients in a variety of ways including face to face, via groups or telephone calls. Also provide support and training for other health care professionals and community workers.
Enhanced Foot Protection Team (EFPT) - Urgent referral to the team for assessment, active management and onward referral to the regional Multi Disciplinary Foot Team (MDFT) in BHSCT.
ACTIVE RISK - those patients with diabetes related foot ulceration.
