Community Nutrition & Dietetic Service

Description

Dietitians provide practical evidence based dietary advice that is specifically tailored to each individual. Through dietary modification the aim is to prevent and treat a range of medical conditions and empower individuals to make appropriate diet and lifestyle choices.

Community Dietitians see patients in a variety of different settings including outpatient clinics, community clinics, care homes and patients own homes. They see patients in a variety of ways including face to face, via groups or via teleconferencing or telephone calls.

As a service they work in variety of specialist areas including:

  • Nutrition support
  • Diabetes
  • Weight management
  • Gastroenterology
  • Home enteral feeding 

Dietitians also provide support and training for other health care professionals and community workers. Increasing the knowledge of other Health care professionals ensures consistency in the provision of evidence based information about food and nutrition.

Referral Criteria

Service is for patients 16+ years (unless a young person with complex needs who remain under CYP)

Refer via CCG or complete Dietetic referral form and email to: ahp.cbu@southerntrust.hscni.net   

Self referrals are not accepted.
