Lurgan Hospital Rehabilitation Unit

Description

Lurgan Hospital is a non-acute inpatient rehabilitation facility. There are 51 consultant led beds which cater for patients with on-going medical and rehabilitation needs that cannot be met in the community setting. Lurgan Ward 1 is a specialist Stroke rehabilitation unit. Wards 2 and 3 in Lurgan Hospital are Care of the Elderly Rehabilitation Units providing rehab to patients 65 years of age or older who are residents of the Southern Health and Social Care Trust.

Referral Criteria
  • Patient must be medically stable prior to transfer to Lurgan Non Acute Hospital
  • Patient (or next of kin) has consented to transfer to Lurgan Non Acute Hospital
  • Patient must have rehabilitation potential that cannot be managed in a community setting such as: own home, nursing home, or residential setting.

Examples include: Specialised rehabilitation equipment that can only be utilised within a NAH setting. Requirement for intense therapy input which can only be provided by a specialist inpatient rehabilitation team of Physios, OTs and SLT staff. Patients requiring review of current medication due to issues with polypharmacy, adherence or compliance.

  • Patient who is ≥ 65 years of age
  • Resident of the Southern Trust
  • Ward 1 Stroke Unit Lurgan – All patients requiring inpatient post-stroke rehab and medical review by the stroke team must be transferred to Ward 1 Lurgan Hospital or Level 4 Daisy Hill Hospital.

Exclusion criteria:

  • Any patient awaiting outpatient review or imaging within 48 hours of transfer
  • Patients who are non-weight bearing as per medical advice, e.g. lower limb fractures in which the patients WB status will not change in the next 1-2 weeks UNLESS there is a clear rehabilitation goal agreed after consultation with the Rehab Assessor and AHP Leads and ICS fracture pathway is not appropriate.
  • Insulin Dependent Diabetic patients who have ongoing unstable blood glucose levels or are requiring frequent adjustments to their insulin doses.
