Multidisciplinary Adult Community Stroke Rehabilitation scheme for service users with a confirmed diagnosis of a Stroke and active rehabilitation goals

• Over 18 years of age
• New diagnosis of acute Stroke within past 6 months
• Resident of SHSCT
• Active rehabilitation goals
• Able to engage actively in rehabilitation
• Consent for CST referral gained

Exclusions:

• Under investigation or confirmation of a differential diagnosis
• Medically unstable
• Unable to engage in rehabilitation
