Steps to Wellness

Description

Steps to Wellness offers group interventions to anyone with mild to moderate mental health difficulties including, but not limited to depression, anxiety, Obsessive Compulsive Disorder, Social Anxiety Disorder, Health Anxiety, Panic Disorder and Perinatal low mood.

GP’s should refer via CCG to MH single point of access, where the referral will be triaged based on the clinical information provided by GP.  It is therefore imperative that as much relevant information is provided on the referral as this will assist triage team in ensuring patient is directed to the most appropriate service within in Mental Health services.

We also have an inter team transfer which enables step up & step down from & to Steps To Wellness if service user’s clinical presentation suggests that their needs would be more appropriately met by a different team.

Referral Criteria

18 and above – no upper age limit

GP in local area + H&C number

MILD TO MODERATE

  • Depression
  • Depression in the context of relationship issues
  • Generalised Anxiety Disorder
  • Obsessive Compulsive Disorder
  • Panic disorder/Agoraphobia
  • Post Natal Depression
  • Psychological impact of long-term health conditions
  • Sleep difficulties
  • Social Anxiety Disorder
  • Consideration of social functioning ie ability to maintain employment.

Social Factors:

  • Stress related to work
  • Stress related to lack of employment/redundancy etc.
  • Stress related to lifecycle or other transitions
  • Other related stress issues

Risk to self or others:

  • Low risk, stable over an extended period of months

Readiness to engage in psychological intervention:

  • Motivation, commitment, and ability to prioritise attending regular weekly appointments.
  • Motivation and Interest in developing new and more helpful patterns of managing one’s own wellbeing.
  • Desire to introduce change to their own behaviour, relationships, beliefs and emotions
  • Self-responsibility for contributing to change.
  • The ability to engage in psychological interventions.
