Community Heart Failure Nurse

Description

This service manages patients with heart failure, for medication optimisation and as a point of contact for patients and carers in the event of decompensation. The Heart Failure Nurse Specialist manages patients with impaired left ventricular systolic function or severe valvular problem. They will also discuss patients with the consultant or GP who do not fulfil these criteria, but who may benefit from a short period of input if they are overloaded and need diuretic management. Please note if patient is required to be seen urgently at home, we will aim to assess them within 48 hours, routine assessments at home will be undertaken within two weeks.

Referral Criteria
  • The HF nurse will accept all those with signs and symptoms of HF who have a cardiologist diagnosis, regardless of whether it is HFpEF/HFmrEF or HFrEF (i.e. accept all HF patients).
  • Patients without signs and symptoms who have > moderate impairment will also be accepted
  • Severe valvular dysfunction may be accepted into the service after discussion with HF consultant
