Intermediate Care

Description

Multi- disciplinary, short term rehabilitation and focus on client remaining / returning to own home. Reduce. Prevention of hospital admission or to provide input to allow client to remain safely in own home. Arrange short term placement in residential/ nursing home if client is unable to remain in own home (for > 65s). the need for long term help at home or long term care. Focus on frailty.

Referral Criteria

Patient/client must have recent deterioration in ability; deemed medically fit for therapy input at this point; anticipate input needed from an Occupational Therapist and Physiotherapist; have patient consent; be a resident of Southern Trust; and will be normally, but not exclusively be over 65.
