Community Parkinson’s Service

Description

This is a Nurse Led Service for patients with a confirmed diagnosis of Parkinson’s Disease who reside within the Southern Health and Social Care Trust. Patients can be seen in community clinics, including face to face appointments and telephone appointments. If housebound a home visit can be facilitated. The aim of the service is to promote self-management of Parkinson’s disease.

Referral Criteria
  • Confirmed diagnosis of Parkinson’s disease requiring input with symptom management.
  • Confirmed diagnosis of Parkinson’s disease requiring advice on medication management.
  • New diagnosis of Parkinson’s disease requiring education to promote self-management of the disease.
  • Must reside within Southern Health & Social Care Trust boundaries

Exclusion

  • Patient currently in-patient in an acute hospital setting.
  • Parkinson’s plus disorders
